cut selects fields without interpreting the record as a spreadsheet. Give both the delimiter and field number. tr then changes characters in the stream; it doesn't select records or remove duplicates. A region repeated in two rows can represent two separate deliveries, so keep both unless the request says otherwise.
Build the extraction first, inspect it, and add case conversion only after you know the right field is flowing through the pipe. For cut -d, -f2, the comma is the separator and 2 selects the second field. This handles the simple delimited records supplied here; a comma inside a quoted CSV value needs a parser that understands CSV quoting. Character conversion changes east to EAST while preserving the stream's row order and repeated values.
